Use the Assessment Center located in DODD's online Apps to request the State-Funded Behavior Rate Add-on for routine homemaker/personal care and community respite services.
The add-on applies only to homemaker/personal care, including daily rate applications and community respite for people enrolled in Individual Options, SELF and Level One waivers.
The person enrolled in the waiver, their SSA and other members of their team should determine if the person meets the criteria for the behavior add-on based on requirements set forth in OAC 5123-9-30. The team should help to complete the questions within the assessment center and county board staff can submit the completed information to DODD. It may be helpful to note that there is also a county-board funded medical add-on outlined in the same rule.
Members of the team should be involved in the decision about whether or not the criteria for a behavior rate add-on has been met. If the board does not agree with the behavior rate add-on, but it is requested by a member of the team, the form should still be submitted and with an indication that the board is not in agreement. A county board cannot decline to submit a referral on the person's behalf.
County boards should continue to fund any behavior add-ons currently in place. The state-funded behavior add-on only applies to people who do not have a behavior add-on in place prior to January 1, 2016.
An approval of the rate is an approval for the waiver span, so the add-on rate will need to be reauthorized annually.
When completing information for this rate within the Assessment Center, please note the following.
- County boards of developmental disabilities do not need to complete the homemaker/personal care and community respite units section for people enrolled in Level One and Individual Options waivers
- County boards do not need to complete budget adjustments after initial DODD authorization, the MSS app will capture those costs.
- Boards do need to fill in the units of community respite for people enrolled in the SELF waiver, as MSS does not do the calculations for these units or unit types. County boards will inserts the dollar amount associated with service code.
- County boards need to fill in budget adjustments after the initial DODD authorization for people enrolled in the SELF waiver because MSS will not be able to capture the costs.
