Providers of services to people with developmental disabilities may be self-employed (called independent providers) or agencies. DODD certifies both agency and independent providers of services. Applicants must meet qualifications, pay a non-refundable fee, and include supporting documentation with their application.
An independent provider or agency is prohibited from providing any service until certification is obtained from DODD.
Initial waiver certification applications for independent providers continue to be accepted by DODD through PSM at this time.
DODD has stopped accepting applications for Initial agency certification as of 03/14/2022. This process will resume in July of 2022.
