Recently, the Ohio Department of Health made changes to the Ohio Nurse Aide Registry. The registry now has a new home on the internet and can be accessed at https://nurseaideregistry.odh.ohio.gov/Public/PublicNurseAideSearch. Please update your internet bookmarks and records to ensure that you are going to the new web address.
We have reached out and have been informed that the site currently presents an error message when no matches are found in the system. If an applicant is not on the registry, the search will bring back the message you see below. You should print or copy the result page for purposes of documenting that the nurse aide registry search was completed. In addition to running the registry report at this website address, you can also download a CSV file at https://nurseaideregistry.odh.ohio.gov/Public/PublicAbuseListing to verify that the person is not on the nurse aid registry. We are recommending that you place a notation on the printed results page to confirm that you verified the error message on the CSV file including the date and time.
If an applicant is on the registry, the results come back with a report that looks like this:
If you have any question regarding a particular applicant within the registry, we recommend reaching out to the Ohio Nurse Aide Registry team directly:
(800) 582-5908 (in-state)
(614) 752-9500 (out-of-state)
Fax: (614) 564-2461
This change did not affect the Nurse Aide Registry information obtained via the ARCS system. DODD will update providers if additional information is received in regards to additional updates or changes with the nurse aide registry.
