LOC App

Use this app to enter and track Level of Care assessments needed to establish a developmental disabilities level of care.

County board of developmental disabilities staff, intermediate care facility staff, or a person designated by DODD may administer the LOC assessment after completing department-approved training.

Completing and Submitting NICS

Notice of Individual Change in Status is also known as a NICS. When a person has a developmental disabilities level of care and is enrolled in a home and community-based services waiver, is a resident of an intermediate care facility (ICF) or a developmental center (DC), and they experience a change in status, the county board or the ICF or DC use NICS to inform DODD of the change. 

This information is for county board evaluators who are completing and submitting a NICS.

  • Log in to DODD Portal to access DODD applications. Select “LOC/IDS.”
  • The “Evaluator Dashboard” will be visible.
  • To access a list of active individuals in your county, select “Individual” from the drop-down menu next to IDS/IMAGINE.
  • Search for the person by typing their name or DODD number into the search box and hit enter or click the magnifying glass.
  • The “individual record” will appear if the person’s records exist in your county.
  • Double click “individual record” to open the person’s record.
  • Click the arrow next to "Information" above the person's name, and select "LOC."
  • Click “NICS” to view all the NICS for a person.
  • Click “+" to add a new NICS.
  • Add a name for the NICS and Type of NICS (Options = Suspension/Restart of Waiver Payment, Disenrollment/Withdrawal Waiver Program, and Change of DODD Waiver). Select which type of NICS you want to submit.
  • Enter all required information and save at any time.
  • Once ready to submit, select the checkbox next to “Submit” and click “Save.”
  • That will submit the NICS to DODD.
  • All fields on NICS will be read only after being submitted. The lock symbol will then appear next to all the fields, and the user cannot edit anything.

Level of Care Date Change

Log in to DODD Portal to access DODD applications. Select “LOC/IDS.”

The “Evaluator Dashboard” will be visible.

To access a list of active individuals in your county, select “Individual” from the drop-down menu next to IDS/IMAGINE.

Search for the person by typing their name or DODD number into the search box and hit enter or click the magnifying glass.

The “Individual Record” will appear if the person’s records exist in your county.

Double click “Individual Record” to open the person’s record.

Click the arrow next to "Information" above the person's name, and select "LOC."

Click “LOC Date Change.”

Click “+” to add a new LOC Date Change. Note: The person needs to be enrolled before you can submit a date change request to DODD.

General information and the person’s current waiver LOC span will be displayed.

Enter the proposed end date and the reason for date change. Complete the attestation and submit the date change form to DODD.

*Remember: Complete the redetermination if the new proposed end date is within the next 15 days.  You will not receive any alerts because the date change has occurred too close to the redetermination requirement.
