​What is the Provider Certification Wizard?

The Provider Certification Wizard is an online certification tool that enables providers and potential providers to navigate through the certification process quickly and with ease. Providers will now be able to log into the Provider Certification Wizard in order to:

  • Apply for initial certification as an independent provider or agency
  • Add services to existing certification
  • Renew existing certification
  • Make changes to existing certification
  • Pay for certification and re-certification online

Who should use the Provider Certification Wizard?

Any person or agency that provides services to individuals with developmental disabilities must obtain certification from the Ohio Department of Developmental Disabilities, and keep their certification current. This includes providers applying to deliver Medicaid Waiver Services under the Level One and/or Individual Options (IO) Waivers, as well as non-Medicaid funded services. Please note, it is necessary that all providers and potential providers have a functioning email address as this will be the primary way that DODD will contact you. There are many companies which will provide you with an email address for free.

*Please note that becoming a certified provider for waiver services does not mean that the provider is an employee of the State of Ohio, the Ohio Department of Developmental Disabilities, or of the provider’s local County Board. Providers are independent business owners who are certified to provide services to individuals on a Level One or IO Waiver.

User Account Creation: Providers who already have a security login and user name to DODD Applications should be able to begin an application for renewal. If you are new or do not have an existing login and user name, please create a provider user account.
