Provider Recertification

The Provider Certification Wizard (PCW) is being re-platformed to a new system titled Provider Services Management (PSM) on December 4, 2017.  Any applications started in PCW but not fully submitted (fees paid) by December 1, 2017 will not be saved. 

If your application is not submitted by 4:00 pm on Friday, December 1, 2017, your application information will be lost and you will need to create a new application in the new system.

The department is excited to offer this newly re-platformed application format and look forward to your feedback. 

Independent providers renew their DODD provider certification every three years by completing the following steps:

Step 1: Gather your documents

You will need the following documents:

  • Current report from the Bureau of Criminal Identification and Investigation (BCII): Your County Board can assist you in finding local organizations that offer BCII and FBI criminal background checks 
    Find more organizations that offer Web Check  on the Ohio Attorney Generals' website.
  • First Aid Certification: Valid American Red Cross or equivalent certification in First Aid
    Find an American Red Cross First Aid training near you.
  • CPR Certification: Valid American Red Cross or equivalent certification in CPR
    Find an American Red Cross CPR training near you.
  • Completion of Annual Required Training: Evidence of completion of annual training on MUI, Client Rights, AND provider's role and responsibilities with regard to services including person-centered planning, community integration, self-determination, and self-advocacy.

Step 2: Log in to the Provider Certification Wizard to complete Recertification.
