Reminder: ICF ODDP, IAF Certification

If you require access to the Ohio Developmental Disability Profile (ODDP) portal and do not currently have an account with DODD, please register here as a provider/ICF evaluator.

You will also be required to complete the ODDP training module and pass the post-test with at least an 80 percent in order to gain ODDP portal access. Access the ODDP training module through DODD MyLearning.

ODDP assessors who have been granted access to a provider by the provider CEO or delegator are now able to attest to ODDPs for residents and submit significant change of condition requests within the ODDP portal. The ODDP facility certification dates follow those of the Individual Assessment Form (IAF), with the initial submission being due 15 days following the reporting period end date (RPED) and a 30-day grace period following.

Remember that providers must continue to complete the IAF in conjunction with the ODDP for each RPED going forward until official notice is given to discontinue the IAF.
